Criteria for Ohio Honoree

• 3-years of publicly open recovery

• Can demonstrate the impact of their “out-loud” recovery 

• An Ohio Resident 

• Must be a person in recovery from SUD or the co-occurring condition SUD & SMI

WHO ARE YOU NOMINATING?

Is the nominee an Ohio resident?
Is the nominee a person in recovery from a substance use disorder or in recovery from SUD and a co-occurring behavioral health condition?
Has the nominee demonstrated public recovery for at least three years?
If the person you are nominating is not selected this time, do you still want them to be notified of your nomination of them? 
Does the nominee know you are nominating them for this honor?
